Understanding the CCC Plus waiver — and how to get help applying.
What Is the CCC Plus Waiver?
Virginia's home- and community-based long-term services and supports program.
The Commonwealth Coordinated Care Plus (CCC Plus) Waiver is one of Virginia Medicaid's LTSS (Long-Term Services and Supports) programs. It helps individuals with disabilities, chronic illnesses, or age-related needs receive the care and support they require while remaining in their own home or community, rather than in a nursing facility or other institution.
Waiver services can include personal care, respite care, and other supports — delivered either through an agency or, for those who want more control over who provides their care, through the consumer-directed model. Under consumer direction, the individual (or their Employer of Record) selects, hires, and supervises their own attendant, with a Services Facilitator like Ascension supporting the process every step of the way.

Three steps to find out if you or a loved one qualifies.
Request an LTSS Screening
Contact your local Department of Social Services (DSS) to request a Long-Term Services and Supports screening. You do not need to have Medicaid or file a Medicaid application to be screened. A Community Based Screening Team — a social worker and a health department nurse — will meet with the individual (and a family member or caregiver, if available) to assess the need for waiver services. If the individual is hospitalized, this screening can often be completed by a hospital discharge planner.
Apply for Medicaid
To have waiver services paid for, the individual must also be eligible for Medicaid. You can apply online at CommonHelp.virginia.gov, by calling the Cover Virginia Call Center at 1-833-5CALLVA (1-833-522-5582; TDD: 1-888-221-1590), or by requesting a paper application — along with Appendix D, specific to long-term services and supports — from your local DSS office.
Select Waiver Services
Once approved for both Medicaid and the CCC Plus waiver, the individual chooses the services they want — including consumer-directed services — and the screening team helps connect them with providers in their area, such as Ascension Community Services for service facilitation and case management.
