Our Story
How we started and where we're headed.
Ascension Community Services provides consumer-directed services to individuals enrolled in Virginia's LTSS (Long-Term Services and Supports) Medicaid waiver — helping people with disabilities and chronic health needs get the care they need while staying in their own homes and communities, rather than in an institutional setting.
Under the consumer-directed model, the individual — or their Employer of Record — chooses, hires, and supervises their own attendant, giving them far more say over who provides their care and how. Ascension supports that process from start to finish — completing the required assessments, building each person's plan of care, and providing the ongoing service facilitation and case management that keeps everything running smoothly and in compliance with waiver requirements.
