Consumer-Directed Attendant Documentation Form — used weekly to record the services each attendant provides.

Consumer-Directed Individual Comprehensive Training Form — completed by your Service Facilitator during required training.

Individual Selection of Consumer-Directed Services — the service agreement signed before you begin employing an attendant.
